A copy of the form and check must be mailed to the above address.

All fields are required except for delegates if not applicable. Only items with an asterisk* will appear on the PFLI Clubs web page or on handouts.

The dues is $70 for clubs with 30 members or less and $80 for clubs with 31 or more members. Checks should be made out to the PFLI.

The dues is required by the end of September. Any club that has not paid by the second competition will be banned from further competitions until the dues is paid with no competition make-ups allowed.
